Arrival
Arrive early & check in
Please arrive 15 minutes before your scheduled appointment to complete check-in. Be prepared to share any recent changes to your phone number, address, email, or insurance coverage at the front desk.
Medications
Bring your medications
- All medication bottles — prescriptions, over-the-counter drugs, vitamins, and supplements
- A written list of current medications for your records
- A note of any medications that need to be refilled

Prepare your questions
Write down any questions or concerns ahead of time — especially anything you'd like to discuss regarding recent lab tests or diagnostic results.

Stay current on immunizations

- Shingles & RSV vaccines available — ask about age-appropriate timing
- Bring records of vaccines received elsewhere so we can update your chart
